lets break that down
1. No hand washing between patients
2. Administration of a medicine drawn up by another and not witnessed by the administrator or at a minimum out of their line of sight.
3. the 5 rights of administering a medicine. None adhered to.
4. A needle designed to administer a medicine to the sub cutaneous region (orange hub), of the skin was used to administer a medicine to a muscle which is much deeper and requires a longer needle (blue hub), especially as Mr Morrison is carrying a lot of subcutaneous FAT. The orange is finer so it probably hurt less.
5. Yet to find out what the dose is but it’s pretty uncommon to administer an adult injectable in a 1 ML syringe. Usually a 2 ML.
6. That doctor is medically negligent in that the location of the injection into the deltoid muscle was contraindicated due to a nerve sitting in that region. It should be the upper outer quadrant of the muscle.
7. No-one EVER inserts the entire length of the needle to the point where the hub makes an indentation into the skin.
