Death Rates From Infectious Disease Fell Dramatically Before Most Vaccines Existed. That Part Isn't Controversial. Clean water, sewage systems, better nutrition, and less crowding cut mortality from measles, scarlet fever, typhoid, and tuberculosis long before any vaccine for them was available. Mortality and incidence are different measures, and conflating them distorts the history in both directions. The honest accounting gives sanitation and nutrition the enormous credit they earned — and it's also the reason public health should be investing in those foundations now, not only in the products that generate revenue.
