I've been watching a lot of Mayday disasters on YouTube and the causes range from sudden mechanical failure, bad maintenance, pilot error and worst of all company error. There was one crash where the company installed an upgrade where the plane would increase power and vertical climb if it detected a reduction in thrust during take-off even though the pilot manually throttled back because the plane was climbing to steeply. Every time the pilot tried to get control by forcing the nose down, the computer in the plane would force a climb. Finally, it climbed so vertical it lost lift and the plane plummeted to the ground killing everyone on board. The pilot would have had to disengage the upgrade manually to override, which no one seemed to inform the pilots nor upgraded the manuals.
