I installed Master Volume Changer (app) from OrchardKit on macOS Sonoma and thought it was broken: the UI worked, shortcuts fired, but system volume barely changed. I chased audio settings and hardware before realizing macOS never gave it Accessibility access. No warning, no prompt. Once I enabled that permission and restarted the app, everything worked instantly.
